1.Carrier truck information–A table template to input all your carriers and owner operators and their information (This is mainly for dispatching companies who work with other carriers)

 

2.Equipment List–Insert all your trucks and trailer information here

 

3.Load board/Truck scheduling template–List your current trucks locations along with rates and it will calculate the weekly gross for each truck and total gross revenue for all trucks. Make sure to only click once in cells with formulas. If you double click on it, hit ENTER. Use colors to mark the status of the truck.

 

4.Owner Operator Settlement Template–The “Amount” rows will automatically add the gross payment and expenses fields will deduct expense from the “total gross.” All dollar amounts will go in the “I” column. Adjust the commission cell percentage “I26” by double clicking on it. Default commission is set to 10%. To change it to 6% (for example), double click on cell “I26” and enter =SUM(I22*0.06). Hit ENTER after you are done to save it.

 

5.Company Driver Settlement–Calculate your driver's pay. Enter the pay per mile amount in cell “E16” and number of miles driven in cell “A16.” Enter deductions in column “I”. It will calculate the total pay automatically.

 

6.Trip Report –This is for your drivers to report each trip back to you. This is usually a requirement from the State's IRP

 

7.Blank Fillable BOL–Use to when you need it and print out copies for your drivers in case a shipper does not provide a BOL.

 

8.Company Handbook–Insert your company name in __________ spaces and have your drivers sign it. Adjust it according to your policies, save it in your driver qualification folder and give a copy to your driver.

 

9.Owner Operator Agreement–Have all your drivers sign this agreement, no matter if they are company drivers or O/Os. Have them keep a copy in their trucks and keep one copy for yourself. Insert your company name in _______________ line.

 

10.Safety planfor your company in case DOT or a broker asks for one.

14.Driver Qualification (DQ) folder cover –Glue it to the outside side of the physical folder and add driver name.

 

15.DQ Folder content list–Glue it to the inside left side of the physical folder (For your reference and to keep track of DQ folder content)

 

16.DQ Truck Template–Glue it to the outside front side of your truck folder. Add truck info.

 

17.DQ Trailer Template–Glue it to the outside front side of your trailer folder. Add trailer info.

 

18.Driver Company info–Type in your company information that the driver needs. Attach it to the front cover of the Handbook folder.

 

19.Vehicle Service Status and Due Report–Keep a copy in each truck and trailer folder. Keep track of FMCSA DOT inspections and when the next inspection is due alongwith any repairs. Include dates and service descriptions.

 

20.New Drivers Check Form–Keep track of steps needed to take when hiring a new driver.

 

21.On Duty Hours of Service Check Form –Keep in DQ Folder. Driver must complete starting with the current date,going back 8 days.

 

22.Driver's Road Certificate–Fill this form out after having a CDL holder ride with the new driver for at least 30 miles and take notes abouthis/her driving skills before dispatching the first load.

 

23.Fair Credit Reporting Act–Keep in DQ Folder.
